Person Centred Planning

What is a Person Centred Plan?

A Person Centred Plan is an event that brings together people who are important to you to listen to what you want from your life.

What does a Person Centred Plan do?

It is something that gives you control over your life. Using one of four different types of plan you can identify the things that you want to change in your life and make plans about how to make those changes.

Why would you want a Person Centred Plan?

A Person Centred Plan will help you think about your future.

By having a Person Centred Plan everyone involved in your life will know how you want to be supported.

Your Plan will help you to make changes or to make sure that the things that you like will stay the same.

How do you get a Person Centred Plan?

You can use your individual budget to pay for the time that is needed to develop your plan, which on average is 3 - 4 hours. ID will provide you with your own personal facilitator to meet with you to find out what you want from your plan. This could be planning to move house, make another big change or to find out about how you can be more involved in your local community. 

The choice is yours, because its all about you.

On the day of your Plan your facilitator will stand up and make sure that everyone listens to what you have to say. They will record everything by writing and drawing, and will involve you fully in how you develop your plan. You can talk about what you want to change, and can plan how to make those changes and identify who will be responsible for helping you to make those changes.

What happens next?

Your facilitator will make sure that everyone knows what they need to do and when they need to do it by. They will then arrange another meeting with you and all of the people involved in moving your plan forward, to make sure they have done what they said they would and that everything is working out well.
